Social Determinants of Health Maps

Where you live can affect the quality of your education, housing, social interactions, economic status, access to healthy foods and physical activity -- all of which are important to your physical and mental health. These types of community conditions are referred to as social determinants of health.  This series of maps displays the regional patterns of both social determinants of health and health outcomes or health behaviors in the United States.
